Over the last 2 years have you been referred for specialist tests or treatment by adult social care, mental health services, your GP, the hospital, or any other service? If so, please take 5 minutes to complete our survey. 

This survey is about people’s experiences of a recent referral. The information you share with us helps us to spot trends to identify areas for improvement.

5. How long did you have to wait between being referred (e.g. by the hospital/community clinic/adult social care/mental health service/GP and getting specialist treatment?
You might want to tell us about any delays to the referral, if the referral was lost or rejected, and / or whether you had to chase anyone up for information about the referral. You may also want to tell us about the communication from your GP or the hospital / community clinic, any support they gave you, etc. You might want to tell us about any positive and negative impacts on your ability to do your day-to-day activities, your ability to work, your emotional wellbeing, your relationships, etc.

Using your story for media

To promote our campaigns we use people's stories to show what needs to change. The media (newspapers, radio or TV) ask if we have people who can their story when we publish a press release about our research.
